Where It All Started: Osseointegration
In the 1950s, Swedish researcher Professor Per-Ingvar Brånemark was studying blood flow in bone using a titanium optical device implanted in a rabbit’s leg. When the study concluded, he discovered something unexpected: the titanium had fused so completely with the living bone that it couldn’t be removed. He called this process osseointegration — and it became the foundation of every modern dental implant used today.
In 1965, Brånemark placed the world’s first titanium dental implant in a human patient. Years of further research followed, and in 1978 he partnered with a Swedish company to bring the technology to the wider world. That company would eventually become Nobel Biocare.
The Birth of All-on-4
In 1998, Portuguese dentist and researcher Dr. Paulo Maló, working on research funded by Nobel Biocare, developed a new approach for patients with extensive tooth loss. Rather than placing six to eight implants per arch — the standard approach at the time — Dr. Maló found that four implants, with the two rear implants angled strategically, could support a full arch of fixed teeth. This angled placement allowed the implants to anchor in denser areas of bone while avoiding anatomical structures like the sinus cavities and nerve canals, often removing the need for bone grafting altogether.
This became the All-on-4 protocol — a name and technique closely tied to Nobel Biocare implants specifically, since the concept was developed and refined using their system. It’s part of why we use Nobel Biocare implants at The Smile Designer: it’s the system the entire protocol was originally built around, backed by over two decades of published clinical research.
Complex All-on-4 Cases: When Melbourne Patients Need More Than the Standard Protocol
Standard All-on-4 works well for many patients, but not every case is straightforward. Some patients have experienced significant bone loss, previous trauma, or long-term denture wear that has changed the shape and density of their jaw. For these more complex situations, additional techniques may be needed.
Zygomatic Implants
For patients with severe bone loss in the upper jaw, standard implants may not have enough bone to anchor into. Zygomatic implants are longer implants that anchor into the zygomatic bone (the cheekbone) instead, which is typically much denser and less affected by the bone loss that occurs after tooth loss. This can allow some patients to avoid extensive bone grafting procedures.
Pterygoid Implants
Similarly, pterygoid implants anchor into the pterygoid plate, a bone structure toward the back of the mouth. These are typically used to support the rear of an upper arch in cases where there isn’t enough bone in the usual posterior implant positions, offering another way to achieve a stable, fixed result without always requiring bone grafting first.
Sinus Lifting
In the upper jaw, the maxillary sinuses can sit close to where implants need to be placed, particularly if bone height has reduced over time. A sinus lift procedure raises the sinus membrane and adds bone graft material underneath it to create enough height for an implant. This can be done in two main ways:
- Internal (crestal) sinus lift: A more minor procedure performed through the implant site itself, typically used when only a small amount of additional height is needed.
- External (lateral window) sinus lift: A more involved procedure accessing the sinus through the side of the jaw, generally used when a larger amount of bone height needs to be added.
Bone Grafting and Augmentation
Where a patient doesn’t have enough natural bone to support implants in the standard way, bone grafting can help rebuild that structure. Graft material can come from a few different sources:
- Synthetic or donor graft material (“fake bone”), which acts as a scaffold that the patient’s own bone gradually grows into and replaces over time.
- The patient’s own bone (autogenous graft), taken from another site — this might be a nearby area of the jaw, or in more significant cases, another part of the body such as the hip.
